PURPOSE OF THE EVALUATION
A functional evaluation is used to to establish the following:

Current condition, symptoms, treatment and response as well as other treatment options available

Current functional abilities and impairments
-
Potential for rehabilitation, treatment or training in order to improve her overall function
-
Exploration of adaptations and reasonable accommodations
PURPOSE OF THE REPORT
The findings are compiled into a report in order to provide the medical scheme with information pertaining to the function of the client to use in the feedback report and facilitate effective recommendations and management of the case
EXECUTION OF THE ASSESSMENT
The assessment can be conducted as a home visit, if appropriate/required but otherwise carried out at the practice. It normally takes 1-2 hours. Standardised tools are used if relevant but generally functional testing is preferred. Clients are requested to bring a list of medications, any medical documents, assistive devices (glasses, wheelchair, hearing aids, crutches etc.) and to wear comfortable clothing.
